Important Information for Our Patients:


1. Supplement List Update:

To ensure your appointment runs smoothly, please review and update your supplement list before your visit. You can upload it as a document or send it via portal message. If you bring in or upload a handwritten list, we will charge an administrative fee to have it entered into your chart. Additionally, if your supplement list is not updated prior to your appointment and the practitioner needs to review it after the visit, a records review fee may apply.


2. Billing Notice:

  • Electronic Statements were issued on 12/04/24
  • Paper statements were mailed on 12/20/24.
  • Electronic statements are sent every 3-4 weeks via the portal. If you don’t check your portal often, we may email your statement.
  • Quarterly paper statements are sent via USPS to the address we have on file.
  • To simplify billing, we strongly recommend keeping a credit card on file for automatic billing every 3-4 weeks.
  • Please reach out if you have any questions about billing.

3. Office Hours:

  • Monday & Wednesday: 9:30 AM – 5:00 PM (closed for lunch from 12:30 PM – 1:30 PM)
  • Thursday: 1:30 PM – 5:00 PM
  • Need to speak with office staff on Thursday morning? Please call or message us to schedule a time.

4. Patient Portal:

The portal is a great tool to:

  • Send quick messages to the office
  • Update your account details
  • View your visit summaries
  • Pay your bill

Please note: If you send a clinical question that requires a phone consultation or chart/supplement review, you may incur a fee of $40–$80.


5. Administrative Fees:

As a small office, we charge a $20 fee for assistance with insurance inquiries, diagnostic submissions, or prior authorizations.


6. Prescription Refills:

Please allow up to 72 hours for prescription refills to be completed. Requests submitted on Fridays or weekends may take until the following week to be processed.

Set Username and Password for Patient Portal

Please create a username and password that you will use to log into the Patient portal in the future.

Your username must be at least 4 characters long

Your password must be at least 8 characters long and include at least one number or special character.

The patient portal gives you access to your medical records and lets you securely communicate with your doctors. When you sign up, you will receive an email with instructions for logging in.
